This is a required form to say that as parent/guardian you are allowing your child to attend youth for the upcoming year. Unlike waiver forms for activities, this form only needs to be filled out once per year.  

MOMENTUM JR. HIGH CONFERENCE

Early Bird registration is now open for the Momentum Jr. High Conference!

What is MOMENTUM?: Momentum is designed specifically for middle school students! It is a high-energy event with a focus on leading students into a deeper relationship with Christ through worship and biblical teaching. The day is full of team games and fun activities for them to participate in!

GRADES: 6-8

LOCATION: Bethel Gospel Tabernacle, 1355 Upper Wellington St, Hamilton, ON

EARLY BIRD COST: $100 (register before October 11 to receive the Early Bird Rate)

REGULAR COST: $120 (registration closes on October 31)

ADDITIONAL COSTS: Students will need to bring money to purchase their lunch. They may also wish to bring additional money for snacks, but snacks are optional.
