This is a required form to say that as parent/guardian you are allowing your child to attend youth for the upcoming year. Unlike waiver forms for activities, this form only needs to be filled out once per year.  

YOUTH TAKEOVER NIGHTS @ BRAESIDE CAMP

DATES: July 1, July 8, July 15

TIMES:

• Meet at the church no later than 6PM (unless meeting us at Braeside)

• Service at Braeside starts at 7PM

• Return to Freedom by 10PM (first rides back), or 11PM (last rides back)

For Grades 6-12

***Please note: Your student must be registered to attend each of the Takeover Nights that they plan on attending.***

GRADES: 2026 Graduates of grades 7 to 12
